With targeted direct mail you get to mail to specific people or businesses in geographic areas of your choosing. This gives you the ability to send a personal message.

For example you may want to target turning 65 people by birth month for a Medicare campaign. Saturation mailing is not targeted. It is data of all mailable addresses without any personal information. For example a pizza restaurant may not be bothered about targeting certain groups of people. They just want to get their message to everyone regardless of who they are.
